1. Health insurance usually doesn't cover cosmetic surgeries unless the cosmetic procedure is to correct defects from an illness (ie. not just to look younger). I would contact your insurance plan and ask them what specifically is covered.
2. Not usually unless it's vital to your health. Mine does not cover it but I have had to have plastic surgery on my eyes 3 times so far to remove growth that will interfear with my sight if allowed to continue to grow and my insurance has covered it all 3 times so you may need to speak to a plastic surgeon to see if it is a medical condition like mine.
